Use the registered business name and the branch location your team will operate from. If the pharmacy already exists on Refill, choose it from the suggestions so we add a new branch instead of a duplicate pharmacy.

Login Account

The person who will manage this branch account.
Reva sends the password setup link here after submission.
After submitting, Reva will give you a secure one-time link to set your marketplace password.

The Pharmacy Council license number and document are required. Superintendent details help the compliance team verify the responsible pharmacist quickly.
